Who qualifies for home care?

An individual with a skilled need (i.e., wound care; dressing changes; physical or occupational therapy; teaching needs with new medications; intravenous medication therapy) who is homebound may be eligible for services.

Homebound is defined as someone who cannot leave their home except for necessary appointments (with taxing effort and in need of assistance), i.e. doctor’s appointment. Home care provides a wide range of services and it is best to discuss with your doctor if you or your loved one qualifies for home care. You may also call your local home care agency. Who qualifies for hospice care?

Any individual with a terminal illness may be eligible for hospice care if they are no longer pursuing treatment or a cure.

 

What services are provided for home care and hospice?

From newborns to the elderly, we provide a wide range of care and services. In fact, many treatments that just a few years ago were done only in an institution can now be provided in the home. Our professional staff of caregivers includes:

  • Skilled Nurses
  • Certified Home Health Aides
  • Licensed Medical Social Workers
  • Physical, Occupational and Speech Therapists

How do I get a referral for home care?
In order to be eligible for home care, you or your loved one must obtain the referral from your doctor. If you believe you qualify for home care services, contact your doctor’s office first.

 

How do I get a referral for hospice care?
A patient, patient’s doctor or family member may make a referral to hospice care. If you believe you or a loved one is eligible for these services based on the above criteria, you can call our hospice intake department and request an informing visit. A hospice representative will come directly to you to determine your eligibility for the program.


Who pays for home or hospice care?

Most Medicare, Medicaid and private pay insurers will pay for home care or hospice services if individuals meet their criteria. Our intake department can help you with this information to determine if you or your loved one is eligible. Your particular insurance may not cover certain services, so it’s important to work with your doctor’s office or speak with a home care agency intake department to determine what services you are eligible for.
